korea is fast
japan approved it for both indications a while ago as i understood it, and the approval did not make it cheap
thailand and taiwan come up occasionally and we have almost no member reports from either, so i would not guess

i was wrong about japanese pricing by a factor of two when i quoted it in here, because i was quoting a clinic and somebody else was quoting a pharmacy, for what its worth
