i escalate on symptoms. if the current dose is still doing something i stay on it, and i am not your clinician
how do you decide between holding and stepping when both feel wrong
about the escalation a step is a decision, not a calendar entry. if the current one is working there is nothing to fix, give it four weeks before you judge
not a prescriber but the calendar tells you when you may move and how you feel tells you whether you should, i am not recommending it
