Highlights from this day
  • lipase_lore — im at 7.5 and stuck, did anyone break a plateau without going up 21:49
  • declared_value — how long till steady state, ive read the half life is about a week changed my mind on the four week rule, i think it depends entirely on how the current dose feels 21:53
  • first_month_fi — SELECT was cardiovascular outcomes in people with overweight and existing disease, not a weight trial 22:08
  • karl_fischer — STEP is several trials, so saying STEP said x is usually wrong without a number after it 22:44
  • meal_prep_mick — my Janoshik report on the QSC vial came back 97.4 against a certificate saying 98.1 23:15

held there too

does moving my dose day by two days matter with a seven day half life

anyone compared their 2 vial dosing against the pen increments

CC

the long half life flattens the trough more than people expect, i never felt a day seven dip

changed my mind on the four week rule, i think it depends entirely on how the current dose feels

Trial scheduleWhat most people here do
Step interval4 weeks, fixed4-12 weeks, on symptoms
Holdingprotocol deviationnormal and expected
Top dosereached by designoften never reached
Coming downnot studiedcommon at maintenance
LL

my plateau was the scale stopping while appetite stayed suppressed, which is a different problem

2.4 is the ceiling

Effect of Semaglutide Coadministered with Intensive Behavioural Therapy (STEP 3)
JAMA · 2021
Semaglutide plus intensive behavioural therapy and a low-calorie diet run-in.

does the seven day half life mean the last two days are weaker

seven days ish

same dose day

RT

my 40 vial in 5ml gives 2mg/ml so 0.25 lands on 20 units, thats why i picked that volume

how many weeks before you noticed anything at 12
i held at 5 for 9 months and it was the best decision i made

👀3

half life is about a week so youre roughly four to five weeks to steady state on any new dose

was SURPASS-2 the one that ran to 68 weeks or am i mixing them up

going slow cost me nothing except time and saved me two bad weeks, thats my whole contribution

MP

FLOW was the kidney outcomes trial and it gets quoted in here for things it didnt measure

CV

while im here what did SELECT actually measure, i see it quoted for everything

mine faded too

TO

went 1 to 1.7 too fast and spent two weeks regretting it, dropped back and repeated the step

KF

ok so lost about 6kg on the low doses before i even got to 1, which surprised me, not advice obviously
my plateau was the scale stopping while appetite stayed suppressed, which is a different problem

how long did the first 43kg take for people at the low doses

moving my dose day by a day or two never did anything noticeable for me

STEP is several trials, so saying STEP said x is usually wrong without a number after it

wk 1-4 0.25mg
wk 5-10 0.50mg (held 2 extra weeks)
wk 11-14 1.00mg
wk 15-26 1.70mg (held, long)
wk 27+ 2.40mg

compounded and brand felt the same to me at 2, which is one person and nothing more

DV

held at 12 and lost 33lb over 26 months, slower than the trials and fine by me

restarted after 5 months off at a low dose and the nausea arrived exactly like the first time

TL

licensed ceiling for weight is 2.4 weekly, anything above that is not a licensed dose

the appetite effect faded for me around 5 months and going up fixed it for a while, happy to be corrected

the plateau hit me at 15 weeks and moved again about a month later without a dose change

FM

five week steps worked better for me than four, purely on how the second week felt

🧊1🤝1

steady state takes weeks

FM

went to 2.4 and the extra suppression wasnt worth the GI for me, so i came back down

MP

steady state is the bit people skip, judging a new dose after eight days tells you almost nothing

HL

if youre comparing brand to compounded, dose accuracy in the vial is the variable, not the molecule

the trials titrated on a fixed schedule, most people here dont, worth remembering when comparing
lost about 45kg on the low doses before i even got to 1, which surprised me

MP

unrelated but my dose day drifted from friday to sunday, does that reset anything

my Janoshik report on the QSC vial came back 97.4 against a certificate saying 98.1

does the licensed ceiling for weight differ from the diabetes one

RT

sorry to jump in changed my mind on the four week rule, i think it depends entirely on how the current dose feels

compounded and brand felt the same to me at 12, which is one person and nothing more

give it four weeks

🧪10⚠️2

STEP 1 ran to 68 weeks. thats the one everyone half remembers, anyway

CC

did anyone titrate slower than four weeks a step and how did that go

i was wrong about the ceiling for a year, i thought 3 existed. it doesnt for the weight indication

€55 a month for compounded against what brand costs locally is why most people here use vials
